PROGRAM ENROLMENT
Program enrolment: 3-step process
Providing your patients with an efficient treatment experience is our goal.
1. Confirmation of enrolment
Once completed, the enrolment form is faxed or emailed directly to the PSP.
Patients will receive a welcome call from the PSP 1 business day after receiving the enrolment form.
2. Reimbursement navigation
Performed (when applicable) to explore coverage options (public and private plans) for your patients.
3. Facilitating delivery
The PSP will help facilitate supply and delivery of CAMZYOS to your patient’s preferred location (i.e., home, workplace or pharmacy).
